Clients are asked to print and complete the Student Referral, Client Policy Agreement/Billing Contract and Release of Information. Those requesting sliding scale or scholarship assistance are additionally asked to complete the Scholarship Request form and attach your 1040 (page 1 & 2) for the current year.
